Series: Before Fixing It, I Had to Recheck the Instruction
After the redesign, a record of times when the "fix it" instruction or the document itself was wrong: fixing a dead comment and getting it wrong again, a hypothesis that was only half right, a mistake avoided by asking once more before fixing, "delete it" and "how to delete it" turning out to be different questions, fixing a prompt rule twice when the place that needed fixing was the stage that splits notes, carefully built detection code that, once measured, turned out to have done nothing, and designing a safeguard against over-splitting without recognizing the same safeguard built the day before.

I fixed the LLM prompt rule twice, but the note should never have been split
An LLM-made sibling note kept coming out empty but for links. Two prompt fixes later, the source showed both courses shared one description: merge, don't split.

The code I carefully built to catch LLM language mixing turned out to have done nothing
I built code to detect and fix Korean sentences leaking into LLM-written English notes. It fixed zero. The real cause was a prompt missing a language rule.

I was designing a guard against LLM note over-splitting and didn't see the one that already existed
An LLM proposed splitting one note into five pieces of 20–30 words each. Designing a guard against over-splitting, I found one built the day before.
